The LifeCourse Trajectory comes from the Supporting Families Community of Practice (http://supportstofamilies.org) and provides a practical approach to looking at where you are today, where you're headed tomorrow, and how the decisions you make every day impact that trajectory. What trajectory are you/your family member on? Will it take you where you want to go? How can you use the Integrated Star and Relationship Map to ensure a positive trajectory?
